In theory it is safe to use long term (and I am assuming that means somewhere between years and the rest of your life which is hopefully many decades). The main threat to using doxycycline longterm was always the effect on your GI system through the killing of good bacteria and the building of super resistant bacterial strains. That being said they will be resistant to tetracyclines, which is really not a frontline therapy to any serious infections. Submicrobial dose doxycycline addresses both of these concerns. Also the dosage and the fact that it is absorbed into body tissue minimizes the wear and tear on the liver, Oracea's momograph should tell you what levels are secreted through the liver, it is likely low.

Reality is it has only been tested to 18 months. Further doxycycline inhibits the absorption of vitamin B, metal oxides and vitamin D. These nutrients in turn block doxycycline absorption (this is why you are told not to take dairy, which is packed with these, within 2 hours of doxy). It is important while you are on doxy to ensure you are keeping your vitamin/nutrient levels up. If you take Oracea in the a.m., take a multivitamin in the p.m. or vice versa.

Sorry, I do not know what part of the B Spectrum it impacts. B12 is one of them for sure, but beyond that I can't find any info.
